Actually, there is a precedent for this. In
the 1960's, the First Presidency of the Church called Hugh B. Brown as the 3rd
counselor.
If it is done on that level, I don't see why it can't be done on a ward level, too. But, if
not, calling anyone to some kind of assistant or second secretary sounds good to me. In our ward, our bishopric gave a task assignment to a person
specifically to plan the quarterly activities for Primary.

My husband is the bishopric counselor over primary & I ran this past him this morning. He said that ultimately, you would need to get approval from
your bishop who would need to get the approval of the stake president. The
only time I have ever heard of this being done is when President Benson became physically incapacitated and they called a 3rd counselor.
